Organization | Manager | Coach

If an organization does not have a team, say A level or AA level, and wants to register one, how does that evolve into them having one?

Does the Organization itself look to add a team?
If yes, do they source a Manager 1st or a Coach?

Does a Coach look to start a team?
If yes, How does he or she approach the Organization?

Does a 'Manager' look to start a team?
If yes, does he line up a potential Coach and then approach the Organization?

It's just a curiosity as it seems every year teams drop, and teams get added. I see Organizations in the G who have teams at only A, only AA, and sometimes both A and AA, yet missing 1 or the other (or both) at various age levels. Wanted to get a layman's understanding of how it worked.

Also, Once there is a a team in place, who holds the power? Can a Coach oust a Manager? Can a Manager oust a Coach? Can the Organization oust either or both?
